Job summary

Encompass Health Corporation is actively seeking a Case Manager to coordinate patient care from admission through post-discharge. You’ll work with an interdisciplinary team to ensure timely, effective treatment plans aligned with patient goals and payor requirements.

Responsibilities include discharge planning, documentation, and patient education. The role emphasizes collaboration, regulatory knowledge, and proactive problem-solving to optimize patient outcomes in a rehabilitation setting.

Qualifications

• Must possess the ability to independently complete assessments within the scope of practice for one's discipline (RN, SW, OT, PT, ST, etc.).
• CCM® or ACM™ certification required or must be obtained within two years of entry into this position.

Responsibilities

• Coordinate patient care from admission to post-discharge with an interdisciplinary team.
• Establish tentative discharge plans and contingency plans with the team.
• Complete case management addendums and maintain required documentation.
• Review case management reports and plan actions based on KPIs and payor requirements.
• Educate patients and families about rehabilitation and the Case Manager role.
