• Initiate and coordinate comprehensive discharge plans for safe transitions (home or facility).
• Coordinate authorization processes with insurers and address coverage issues.
• Act as advocate, providing emotional support throughout rehabilitation.
• Maintain accurate documentation in patient records (assessments, plans, progress).
