Job summary

Los Robles Regional Medical Center is seeking a Rehab Specialist PRN to support the rehab program by coordinating intake, admissions, and clinical evaluations. You will collaborate with case managers, physicians, and payors to ensure proper patient placement under the Rehab Medical Director's guidance.

The role requires a valid clinical license and at least 2 years of inpatient rehab experience; BCLS is preferred.

Qualifications

• Licensed clinical professional with appropriate state licensure.
• Graduate from an accredited clinical program.
• Minimum 2 years of recent clinical care experience.
• BCLS (AHA) certification preferred.
• Pass FIM credentialing within 90 days of hire and maintain.

Responsibilities

• Assist and coordinate intake and pre-admission screening with documentation of rehab admission criteria.
• Perform on-site and off-site clinical evaluations to determine appropriateness of admission.
• Demonstrate knowledge of rehab admission criteria and identify qualified rehab patients.
• Act as primary contact for external/internal case management and referring physicians; maintain relationships with referral sources.
• Complete face-to-face educational contacts with physicians, case managers and other professionals; serve as an educational resource.
