Job summary

Qualifications

• Vocational/Technical Degree or equivalent combination of education & experience.
• Experience in Acute Rehab or Skilled Nursing preferred.
• Experience with electronic medical records preferred.
• Liaison experience preferred but not required.

Responsibilities

• Assess patients for placement at Reading Rehab Hospital’s Acute and Subacute Transitional Care units.
• Coordinate admissions and transfers per standards and criteria.
• Collaborate with physicians, leadership, and case management to prioritize admissions.
• Evaluate patient needs to determine unit-appropriate placement.
• Promote services and act as liaison between hospital departments, patients, referral sources, and clients.
• Provide excellent customer service to physicians, patients, administrators, and hospital clients.
