A leading healthcare organization is seeking a Registered RN - Care Coordinator for the Cancer Institute, focusing on patient advocacy and care coordination. Position Summary

Position Summary:
Collaborates with the clinical team to identify patients who benefit from care coordination, assessing risk factors and needs for clinical utilization management and preventive care services.
Work location: 3090 Caruso Court, Suite 50, Orlando, FL 32806. Work hours: Monday - Friday, 8:00 AM - 4:30 PM.
Responsibilities include ensuring continuity of care across inpatient, emergency, and outpatient settings, developing relationships with social workers and utilization review nurses, engaging patients and families in care plans, monitoring progress, educating about chronic illness management, and ensuring access to prescriptions and medical equipment. The role involves problem-solving, advocating for patients, facilitating system navigation, participating in clinical outcome measurement, and maintaining compliance with policies and standards.
Qualifications include a BSN (effective July 1, 2024), a current Florida RN license, BLS certification, and at least three years of clinical experience relevant to the patient population.
Other functions include demonstrating analytical and team-building skills, knowledge of growth and development principles, awareness of legal and regulatory issues, and the ability to prioritize independently.
This is a mid-senior level, full-time position in the healthcare industry focused on hospital and outpatient care services.
