Position Summary

Position Summary
Collaborates with the clinical team to identify patients who will benefit from care coordination, assessing risk factors and the need for services such as clinical utilization management and preventive care.

  • Work location: 3090 Caruso Court, Suite 50, Orlando, FL 32806. Work hours: Monday - Friday, 8:00 AM - 4:30 PM.

Responsibilities

Essential Functions

  • Ensure continuity of care across inpatient, emergency, and outpatient settings, including discharge planning and follow-up care.
  • Develop relationships with Patient and Family Counselors/Social Workers and Utilization Review Nurses to engage patients and families, supporting their needs and discharge plans.
  • Monitor patient progress, adjust discharge plans as needed, especially for high-risk patients.
  • Educate patients and families on self-management and the healthcare system.
  • Identify support needs, develop action plans, and facilitate access to prescriptions, DME, and other services.
  • Participate in clinical outcome measurement and population health strategies.
  • Maintain patient safety and compliance with policies, procedures, and standards.

  • Provide age-appropriate care, understanding growth and development principles.
  • Be aware of legal, patient rights, and regulatory issues.

Qualifications

Education/Training

  • Effective July 1, 2024: New hires and transferring staff must have a Bachelor of Science in Nursing (BSN).

Licensure/Certification

  • Current RN license in Florida.
  • Current BLS/Healthcare Provider certification.

Experience

Minimum three (3) years of clinical experience, particularly with the population served in this role.
