Upstate Caring Partners is seeking a Registered Physical Therapist to evaluate and treat individuals across programs, promoting mobility, strength and independence. You will collaborate with educators, healthcare professionals, and families to implement customized plans and contribute to IPP/IEP goals.

The role requires NYS licensure, 1–3 years of experience, and a valid NYS driver’s license with travel between sites. We offer a competitive salary and a comprehensive benefits package.

Qualifications

• Bachelor's Degree in Physical Therapy
• Current NYS Physical Therapy Registration/Licensure
• 1-3 years of related experience preferred
• A valid NYS Driver's License and ability to travel between sites

Responsibilities

• Evaluate and assess individuals to develop personalized, meaningful treatment goals.
• Implement therapeutic interventions to improve mobility, function and quality of life.
• Educate individuals, families, and team members on treatment plans, therapy methods, injury prevention, positioning, range of motion, and more.
• Collaborate with educators and healthcare professionals ensuring alignment with IPP/IEP.
• Maintain accurate and timely documentation per program and regulatory requirements.
• Support departmental operations, including equipment needs, adaptation, and orientation of new staff.
• Participate in agency-wide initiatives that advance our mission and wellbeing of the people we serve.
