Upstate Caring Partners, Inc. is seeking a Registered Physical Therapist to join our Central New York team. You will provide expert evaluation, therapy services, and collaborative care across our programs to help individuals improve mobility and independence.

Must hold a NYS PT license, a Bachelor's in Physical Therapy, and have 1–3 years of experience. A valid NYS Driver's License and ability to travel between sites required. We offer a competitive benefits package and sign-on bonus eligibility.

Qualifications

• Bachelor's Degree in Physical Therapy is required.
• Current NYS Physical Therapy Registration/Licensure is required.
• 1–3 years of related experience preferred.
• A valid NYS Driver's License and ability to travel between sites.

Responsabilités

• Evaluate and assess individuals to develop personalized, meaningful treatment goals.
• Implement therapeutic interventions to enhance mobility, function, and quality of life.
• Educate individuals, families, and team members on treatment plans and prevention.
• Collaborate with interdisciplinary teams ensuring alignment with IPP/IEP.
• Maintain accurate documentation per program and regulatory requirements.
• Support departmental operations and orientation of new staff.
• Participate in agency-wide initiatives to advance mission.
