What does a typical day look like for the Registered Polysomnographic Technologist (RPSGT)?

You will:

  • Overnight Shift. Thursday, Friday, Saturday 6:00 pm - 7:30 am.
  • Direct Patient Care: Estimated 95% of your time is spent in direct patient care.
    • Overnight Nocturnal Polysomnography (PSG) – diagnostic, split-night, and titration studies. Parasomnia montage studies when ordered. Daytime sleep testing: Multiple Sleep Latency Test (MSLT) and Maintenance of Wakefulness Test (MWT). Home Sleep Apnea Testing (HSAT/HST) – device preparation, patient instruction, and post-study data download.
    • Event scoring of sleep studies (respiratory events, sleep states, arousals, limb movements, cardiac rhythm, parasomnias). Preparation of studies for Technician interpretation, including proper labeling, summary sheets, and technical notes.
    • Set‑up, interface, and calibration of PSG equipment (16-channel and 32-channel systems depending on local platform. Operation of computerized non-invasive monitoring system for: airflow and respiratory effort, pulse oximetry, transcutaneous CO2, Plethysmography, ECG, muscle tone, digital video monitoring system. Daily quality control of all sleep-lab equipment. Identify and report suspected malfunctions to Biomedical Engineering and IT support.
    • Apply, monitor, and adjust PAP modalities: CPAP, BiLevel, BiLevel ASV, and BiLevel ST/VAPS. Recognize respiratory events that require PAP pressure adjustment according to AASM titration guidelines. Document PAP tolerance, mask fit, leak, and response.
    • Explain procedures and ensure patient comfort to optimize data quality. Monitor and record significant observations, including: Sleep stages, heart rate and respiratory rate, oxygen saturation, body position, presence/absence of apnea, hypopnea, snoring, nocturnal seizures, bruxism, cardiac arrhythmias, and parasomnias. Recognize artifacts and correct them to maintain valid signals. Identify urgent clinical concerns and respond appropriately: notify supervisor for noncritical issues, activate VA Code Blue procedures when needed, and initiate CPR when required. Maintain patient safety and privacy in accordance with VA/HIPAA requirements. Maintain current BLS certification at all times.
    • Organize, prepare, and complete patient charts, tracings, and technical notes for physical review. Accurate, timely documentation in CPRS/HER systems. Upload and transfer electronic files and diagnostic data in accordance with VA standards. Ensure compliance with VHA Directive 1088 regarding communication pathways for diagnostic results (in coordination with supervising providers).
    • Prepare PAP supply requests (filters, tubing, masks) when required by the Sleep Program Chief. Perform other duties that directly support Sleep Lab operations, as assigned by Sleep Medicine leadership (nonclinical tasks must remain appropriate for an RPSGT scope). Participate in Sleep Lab quality-control measures and maintenance of accreditation standards.
    • Shall report on time for all scheduled shifts as documented by physical presence in the Sleep Laboratory at the scheduled start time. Shift schedules will align with facility requirements as coordinated with the COR or Sleep Lab Chief (e.g., night shifts or daytime diagnostic/testing shifts). Sleep Lab leadership at VA Puget Sound confirms these staffing operational needs in Sleep lab contracted staff.
    • While RPSGTs do not diagnose conditions, they shall ensure all recorded data, scoring results, and technical notes are uploaded and routed appropriately so that supervising sleep Technicians can meet communication requirements outlined in VHA Directive 1088. RPSGTs must follow all established sleep lab reporting workflows for notifying supervising clinicians about abnormal findings requiring urgent review (e.g., possible nocturnal hypoventilation, severe desaturation, dangerous arrhythmias).
    • RPSGTs shall not prescribe, dispense, or administer medications. Consistent with VA policy and DEA prescribing requirements, only credentialed independent practitioners may administer medications. RPSGTs shall adhere to all established facility policies regarding medication environments (e.g., PAP humidifier solutions, oxygen therapy supplies, etc.).
    • Upon completion of sleep studies, RPSGTs shall provide appropriate post‑study instructions such as:
      • Guidance on PAP equipment return and cleaning.
      • Post‑study safety reminders (e.g., drowsiness after MSLT study).
      • Instructions for follow‑up appointments or PAP initiation based on preliminary Technician guidance.
      • All patient education must be consistent with VA Sleep Lab workflow and coordinated with the supervising Sleep Medicine provider.
  • Administrative: estimated 5% of time not involved in direct patient care.
