Las Colinas Post Acute is seeking a Registered Physical Therapist (PT) to join our rehabilitation team in Ontario, CA. This role offers the opportunity to provide patient-centered care within a collaborative clinical environment.

Qualifications

• Active Physical Therapist license in the state of California.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Collaborate with an interdisciplinary team to implement and adjust care plans.
• Supervise and support therapy assistants as appropriate.
• Document evaluations, progress, and outcomes in compliance with regulatory standards.
• Deliver high-quality care with compassion, professionalism, and clinical excellence.
