Job summary

Las Colinas Post Acute is seeking a Registered Physical Therapist (PT) to join our rehabilitation team in Ontario, CA. We offer competitive pay at $52-60 per hour (DOE) and a supportive, collaborative clinical environment focused on resident-centered care.

The role requires an active California PT license and a graduate of an accredited PT program, with emphasis on teamwork and compassionate care. Excellent opportunities for professional growth are available.

Qualifications

• Active Physical Therapist (PT) license in California.
• Graduate of an accredited Physical Therapy program.
• Strong commitment to patient-centered care.
• Excellent communication and teamwork skills.
• Experience in a skilled nursing or post-acute setting preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Collaborate with an interdisciplinary team to implement and adjust care plans.
• Supervise and support therapy assistants as appropriate.
• Accurately document evaluations, progress, and outcomes in compliance with regulatory standards.
• Deliver high-quality care with compassion, professionalism, and clinical excellence.
