As part of a care management team managing complex members, the Community Well Care Coordinator will provide care management for a panel of older adult members with medical and behavioral health needs. Activities will focus on supporting members' medical, behavioral, and socioeconomic needs to promote appropriate service utilization and improved quality of care.

Work Schedule: Monday through Friday, 8:00 am to 5:00 pm

If you reside in Indiana, you will have the flexibility to telecommute* as you take on some tough challenges.

Primary Responsibilities
  • Assist with care management for high-risk members with comorbid behavioral health needs
  • Assess members face-to-face and/or telephonically to complete comprehensive needs assessments, including medical, behavioral, functional, cultural, and socioeconomic (SDoH) needs
  • Develop and implement individualized, person-centered care plans with goals, opportunities, and interventions aligned with the member’s readiness to change
  • Partner and collaborate with internal care teams, providers, and community resources to implement care plans
  • Provide referrals and linkages as appropriate, including internal consults (Housing Navigator, Pharmacy Team, Peer Specialist) and community-based providers (PCP, specialists, medication-assisted therapy)

Required Qualifications
  • Possess one of the following:
  • Bachelor’s degree in social work, psychology, counseling, gerontology, or health & human services
  • Bachelor’s + 2 years of full-time experience with older adults/disabilities
  • Associate degree + 4 years of experience in similar roles
  • 2+ years in long-term support services or working with older adults
  • 1+ years in MS Office (Word, Excel, Outlook)
  • Access to a private, secure home workspace with PHI security
  • Reside in Indiana and have high-speed internet access
Preferred Qualifications
  • Training in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care
  • Experience in team-based care, co-occurring disorders, managed care, bilingual skills
Physical Requirements
  • Ability to move between office and field locations
  • Navigate various terrains for visits
  • Remain stationary for computer work
