Reliant Care Management, LLC seeks Registered Nurses for day and night shifts to provide compassionate patient care. Responsibilities include assessment, medication administration, vital sign monitoring, education for patients and families, and care coordination with healthcare teams.

We are currently seeking RNs for our day and night shifts.

Job Responsibilities and Duties:

• Assess patients’ health needs and develop and implement care plans

• Administer medications and treatments as prescribed

• Monitor patients’ vital signs and report any changes to the appropriate staff member

• Communicate with patients and families to provide education and support

• Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment

• Document patients’ progress and outcomes using electronic medical records

• Stay up-to-date with current research and best practices in nursing

• Participate in professional development opportunities to maintain and improve skills

• Other duties as assigned

Requirements:

• Current unencumbered state license as a Registered Nurse
