Job summary

Advocate Health is seeking an experienced transplant nurse to lead and support care across the transplant continuum. You will coordinate testing, eligibility and donor processes, and deliver pre- and post-transplant education to patients, donors, and families.

The role requires strong teamwork in multidisciplinary settings, adherence to protocols, and ongoing quality improvement to advance outcomes in a fast-paced transplant center.

Qualifications

  • Graduate of an accredited School of Nursing; BSN required or enrolled in BSN program within 3 years of hire/transfer.
  • Current RN license; CPR certification required.
  • 3 years nursing experience required.

Responsibilities

  • Assures that required tests are completed and reviewed per transplant-specific protocols.
  • Analyzes test results and diagnostic studies; prescribes strategies for outcomes.
  • Interprets eligibility criteria to identify high-risk recipients/donors and schedules consultations.
  • Identifies health problems that may increase transplant-related morbidity and mortality.
  • Coordinates procurement of donor cells/organs while complying with HIPAA.
  • Participates in multidisciplinary team meetings to develop a comprehensive care plan.
  • Conducts ongoing evaluation of transplant recipients and attainment of goals/outcomes.
  • Provides education pre- and post-transplant to recipients, donors, and caregivers via 1:1 and group sessions.
  • Seeks knowledge reflecting current nursing practice and futuristic thinking.
  • Assists in development of policies and procedures aligned with standards.
  • Participates in committees and governance and fosters research to improve outcomes.
  • Engages in quality and performance improvement activities.
  • Uses resources to plan and sustain evidence-based transplant nursing services.
  • Promotes evidence-based practice for the Transplant Center.

Job description

Major Responsibilities


  • Assures that required tests are completed and reviewed per transplant specific protocols.

  • Analyzes test results and diagnostic studies and develops a plan that prescribes strategies to obtain expected measurable outcomes.

  • Interprets eligibility criteria to identify high-risk recipients and donors; schedules appropriate consultations to ensure all health needs are met prior to transplant. Communicates eligibility with multidisciplinary team.

  • Identifies and communicates health related problems that may increase transplant related morbidity and mortality.

  • Coordinates procurement of donor cells/organs while complying with HIPPA standards

  • Participates in multidisciplinary team meetings to develop and implement a comprehensive plan of care.

  • Conducts ongoing evaluation of transplant recipients to include effectiveness of intervention and attainment of goals and outcomes.

  • Provides education pre- and post-transplant to recipients, donors, and caregivers through 1:1 consultations and group classes.

  • Seeks knowledge and competence that reflects current nursing practice and promotes futuristic thinking.

  • Assist in development of policies and procedures reflective to current practice/literature/accreditation standards.

  • Participate in committees, shared governance, or facility decision making activities, developing, and nurturing research to positively affect clinical outcomes and promotion.

  • Participates in quality and performance improvement activities.

  • Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective, and fiscally responsible.

  • Promotes evidence-based practice for the Transplant Center treatment intervention.


Minimum Job Requirements

Education

  • Graduate of an accredited School of Nursing required. BSN required. RN candidates for external hire or internal promotion without BSN must be enrolled in a BSN program upon hire/transfer date and must complete program within 3 years of enrollment in the program.


Certification / License

  • Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works. CPR Certification required.
