Job summary

Greater Springfield Senior Services Inc. is seeking a nursing professional to perform on-site assessments and coordinate care for the CAE/State Home Care Program.

Responsibilities include initial and annual assessments, care plan development, and liaison work with providers to ensure comprehensive elder care. The role requires MA RN certification (or Diploma RN with additional community-based experience), strong tech skills, and reliable transportation.

Qualifications

• Current MA RN license with at least 1 year clinical community or long-term care nursing experience.
• Alternative: Diploma RN with 2 additional years of community-based nursing or long-term care direct service experience.
• Proficient with technology for on-site assessments and data entry, including laptops/tablets.

Responsibilities

• Complete initial assessments and annual reassessments for personal care and home health aide clients.
• Develop care plans for personal care clients.
• Prepare and maintain nursing files (paper and electronic).
• Provide consultation on Protective Services, Elder at Risk and Respite Care cases.
• Act as liaison with agency providers and as a medical resource for staff.
• Backup nurse for other medical programs as needed.
• Be an active interdisciplinary team member providing nursing support to case management staff.
• Advocate for elders and assist with information and referrals.
• Requires valid driver's license and reliable transportation; ability to climb stairs.

Responsible for assigned nursing duties for Clinical Assessment and Eligibility (CAE)/State Home Care Program. Will Perform Coordination of Care screenings to determine medical eligibility and all duties required under the Coordination of Care Program and Waiver Program.

• Complete initial assessments and annual reassessment visits on all personal care and home health aide clients, interim visits as needed.
• Develop appropriate care plans for personal care clients.
• Prepare and maintain nursing paper and electronic files.
• Provide consultation on Protective Services, Elder at Risk and Respite Care cases.
• Act as a liaison with agency providers.
• Act as resource person for medical matters to all Greater Springfield Senior Services staff.
• Act as a backup to the nurse of any other medical programs.
• Active member of the interdisciplinary team providing nursing consultation to case management staff.
• Advocate and act as a resource in areas of interest and/or concern for elders.
• Assist with information and referrals.
• Requires valid driver's license and reliable transportation.
• Ability to climb multile levels of stairs.

Education and Experience

• Current MA RN license and at least one (1) year clinical experience and/or nursing in the community or long-term care setting.

Or

• A diploma RN and two (2) additional years of community-based nursing experience or long-term care direct service experience.

Core Competencies

• Ability to utilize technology in the field to complete On Site Assessments including Laptops, Tablets, Personal Hot Spots Devices, WIFI, etc.
• Working knowledge of spreadsheets, word processing and data entry, including Windows, Word, and Excel helpful
• Ability to conduct professional assessments in a variety of home settings in a manner that is respectful and supportive to the occupants of the home
• Ability to work with persons with diverse backgrounds, skills and abilities
• Experience working with disabled persons preferred
