The ideal candidate will hold a CT RN license, have CPR certification, and bring about 3 years of experience in a medical practice, with a compassionate, patient-centered approach and strong teamwork in a collaborative environment.

• Provide office-based nursing care in collaboration with the provider.
• Assess physical and psychosocial needs and environmental factors affecting patients and their families.
• Formulate nursing plan of care and ensure continuity of patient care.
• Demonstrate independent nursing actions based on assessment and problem identification.
• Triage telephone calls and initiate appropriate nursing intervention.
• Assist patients and family members with concern and empathy while respecting confidentiality and privacy.
