Hartford HealthCare in Hartford, CT is seeking a Registered Nurse (RN) for the 94002-HHC 24x7 unit to deliver skilled nursing care across clinical settings in collaboration with the provider. You will assess, plan, intervene, and evaluate patient problems while maintaining confidentiality and a compassionate approach.

This full-time, day-shift role requires CT RN licensure, CPR certification, and about 3 years of medical-practice experience.

Qualifications

• Current CT RN license required
• 3 years of medical practice experience preferred
• CPR certification required
• Experience with medical software preferred

Responsibilities

• Provide office-based nursing care in collaboration with the provider.
• Assess physical and psychosocial needs and environmental factors affecting patients and their families.
• Formulate nursing plan of care and ensure continuity of patient care through appropriate evaluation and intervention.
• Demonstrate independent nursing actions based on assessment and problem identification.
• Triage telephone calls and initiate appropriate nursing intervention in a prompt, precise, and professional manner.
• Assist patients and family members with concern and empathy; respect confidentiality and privacy.
• Travel to other regional offices as needed.

Requirements

Education

• Current state of Connecticut RN license required.

Experience
