• Assess and respond to resident conditions using nursing process.
• Communicate with interdisciplinary team to maintain high standards of care.
• Document care and services per facility policies.
• Complete incident reports and change in condition reports as needed.
• Administer medications and treatments timely and accurately.
• Notify physician of changes in resident condition and document updates.
• Update nursing and medical plans of care as required.
• Ensure all team members understand changes in care plans.
