Mountainside Nursing Home in Mountainside, New Jersey is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct and coordinated care to residents as part of our nursing team.

This role requires a NJ LPN license and at least 1 year of experience. You will administer medications, monitor residents, update care plans, and collaborate with RNs to ensure high-quality outcomes in a fast-paced skilled nursing environment.

Qualifications

• 1 year of experience as an LPN.
• Current LPN license in New Jersey.
• Ability to work in a fast-paced environment with priorities.
• Strong communication and interpersonal skills.

Responsibilities

• Provide direct and indirect care according to each resident's plan of care.
• Assess, observe, and document clinical condition in EMR and notify RN/provider as needed.
• Update or modify care plans reflecting changes in resident needs.
• Administer medications timely and accurately per orders.
• Participate in meetings related to resident outcomes and care.
• Respond to residents, family, or responsible party promptly and professionally.
