Qualifications

• Education: Graduation from an accredited School of Nursing.
• Experience: One year as an RN.
• Licensure/Certifications: Arizona Nursing License or compact state license and BLS; American Heart Association as required.

Responsibilities

• Implements and monitors patient care plans and communicates patient condition to the team.
• Coordinates care across disciplines for well-coordinated patient care.
• Follows physician and nursing orders and coordinates discharge planning.
• Performs initial, re-assessment, and focused assessments as needed.
• Develops care plans with patient and family, sets daily goals, and plans transitions or discharges.
• Administers medications and provides patient and family education.
• Ensures ongoing evaluation of patient status and care plan adjustments.
