Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for weekend visits and Start of Care/admissions in home health settings. The RN will join a multidisciplinary team to deliver skilled nursing and rehabilitative care that keeps clients at home.

Requirements include an active nursing license in Arizona and at least one year of relevant clinical experience. Per diem/part-time/full-time options may be available, with travel across the service area.

Qualifications

• Current nursing license in good standing in the state of Arizona.
• At least one year of hands-on medical experience with adult/geriatric population in acute care, emergency, home health, hospice, ICU, med surg, sub-acute care or rehab.

Responsibilities

• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Care for and educate patients in acute and post-acute home settings, including wound care and IV therapy.
• Triage, manage urgent clinical situations, and conduct medication reconciliation.

Registered Nurse - Weekend RN - Home Health Visits

BAYADA Home Health Care is seeking aRegistered Nurse (RN) to join our East Tucson home health visit team. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. This is a Weekend RN position - visits will include mostly Start of Care / Admissions. Home Health and OASIS documentation experience is required.

This role requires flexibility and the ability to travel throughout the office’s entire coverage area. Per Diem, Part Time, and Full Time schedules available.

How You’ll Make an Impact:

• Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
• Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation , teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

Qualifications for a Registered BAYADA Nurse:

• A current nursing license in good standing in the state of Arizona
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
