The role requires a Bachelor's degree in nursing (or equivalent), an active Ohio RN license, and 2–4 years of direct hospice or home care experience.

Qualifications

Responsibilities

• Maintain a primary caseload for hospice patients.
• Provide skilled professional nursing care to patients and families as prescribed by the physician, compliant with Hospice Medicare Conditions of Participation and Ohio licensure laws.

Status and Shift: Full Time, Part-Time and PRN (No Scheduled Weekends)

Responsibilities

• This role is responsible for primary caseload.
• This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.
