Trinity Health Michigan Chelsea Hospital is seeking a SWAT & ICU RN for the 7pm–7:30am night shift. The role includes mentoring staff, coordinating patient care, and leading in-staff communication to ensure top-quality outcomes.

Full-time, with a 72-hour per pay period schedule and every third weekend and holiday rotation. The position requires a valid Michigan RN license, a nursing degree, and BLS/ACLS credentials, with ICU/ED experience preferred.

Qualifications

• Graduation from an accredited nursing program and Michigan RN licensure.
• BSN within five years or agreement to obtain BSN within five years from hire/transfer.
• Minimum of three years acute care experience; ICU/ED experience preferred.
• Current BLS and ACLS certifications.
• Canadian nurses encouraged to apply.

Responsibilities

• Lead and coordinate day-to-day operations and interdepartmental communication.
• Respond to urgent situations with multidisciplinary teams per hospital protocol.
• Participate as Code Blue Team coordinator when needed.
• Collaborate across ER, ICU, and other units to ensure high-quality care for acute stroke patients.
• Coordinate patient transport and handoffs between units and facilities.
• Mentor new staff in chart review, protocols, and multidisciplinary communication.
• Support staff in problem-solving and advocating for patients and families.

Position Purpose

The SWAT & ICU RN demonstrates expert proficiency in working with complex patients within the appropriate unit's scope of practice and serves as a resource and leader to promote patient safety by relocating the patient to the appropriate location when necessary. The Swat RN has a role in mentoring and supporting newer staff and facilitating communication, assisting in the advancement of Nursing and organizational initiatives, protocols, and quality improvement. He/she serves as a role model for expert, contemporary, evidence based nursing practice, promoting ANA standards of care, Just Culture, and Relationship Based Care.

Full time, benefit eligible (72 hours per pay period - every two weeks)

Every 3rd weekend and holiday rotation required

This position will be working 24 hours per week supporting our SWAT team across the Chelsea hospital and 12 hours per week supporting our ICU unit.

Required Qualifications

• Graduation from accredited nursing program & Current Michigan RN license
• Effective January 1, 2013 all new hires must have a BSN degree or must agree in writing to obtain their BSN within five (5) years from date of hire or transfer to an RN role.
• Minimum of three years acute care experience. Two years related practice experience in Intensive Care clinical area or Emergency Department preferred.
• Current BLS & ACLS
• Canadian Nurses are encouraged to apply

What you will do

• Leads, facilitates, and addresses day-to-day operational issues and communication between nursing and other departments.
• Coordinates response to urgent/emergent situations including summoning of multidisciplinary team members such as: Respiratory therapy, Physicians and/or APPs, Other disciplines that may be necessary, as described in the relevant hospital protocol.
• Participates as a Code Blue Team coordinator when needed.
• Collaborates between the ER, critical care physician team, primary physician team, patient care unit and neurology to provide excellent and efficient care to the acute stroke population.
• Participates in planning, coordination, and monitoring of patient transportation in the acute and critically ill population to interventional and diagnostic areas outside the units utilizing patient handoff among nursing staff members.
• Proactively rounds on the inpatient units (focus on identification, assessment, and intervention for high-risk patients), discussing patient population with charge RN.
• Assists with diagnostic imaging procedures requiring an RN (Bubble studies, MRI for pacemaker patient) when Diagnostic Imaging RNs are not available.
• Mentors new staff in: chart review, protocols, communication with multidisciplinary team to prevent and identify deterioration in patients.
• Coaches, mentors, and supports colleagues to be effective patient advocates. Supports staff in problem solving and utilizing effective communication strategies to collaborate with other disciplines.
• Responds to family requests for individual case reviews utilizing appropriate resources and disciplines. Promotes environment that supports remarkable patient care experience. Role models clinical excellence. Anticipates and addresses patient/family/visitor concerns in collaboration with caregiver, physician, and unit charge RN.
