Job summary

Prestige Care Center of Fairfield is seeking compassionate Registered Nurses to join our skilled nursing team. You will provide routine and wound care, manage medications, and collaborate with the care plan team to ensure quality resident care.

Qualifications include a nursing degree (LPN/RN accepted), and state licensure. Two years of RN/LPN experience preferred. A $10,000 sign-on bonus is offered for eligible applicants.

Qualifications

Responsibilities

• Ensure adequate stock of medications, supplies, and equipment on the unit and notify needed personnel.
• Deliver routine medications and monitor for adverse drug reactions.
• Document medication administration and follow written instructions for medications.
• Observe residents for changes and document accordingly.
• Perform medication-related tasks: vital signs, dose calculations, stocking carts, removing discontinued meds.
• Provide wound care per physician orders and manufacturer recommendations.
• Contribute to care planning and document care problems and changes in the resident's chart.
• Communicate with physicians, supervisors, and residents or families as needed.
• Support preventive measures like turning, pressure relief, nutrition, and toileting care.

Minimum requirements include the following:

• A nursing degree from an accredited college or university, or be a graduate of an approved LPN/RN program.
• Two years of experience as an LPN/RN preferred.
• Eligible to participate in federally funded health care programs.

Must also meet state requirements for relevant licensures or certifications and have no disciplinary action in effect against professional license.

Continuing education in wound care and pressure injury staging required. Certification in wound care preferred, but not required.

Major Duties and Responsibilities of a Registered Nurse

• Ensures an adequate stock of medications, medical supplies, equipment, etc., is maintained on the unit at all times to adequately meet the needs of the resident and notifies appropriate personnel of needs.
• Delivers routine daily oral, inhalation and topical medications to residents
• Observes for any adverse drug reactions and reports any problems to the nurse.
• Records medication administration according to company policies and procedures.
• Follows written or verbal instructions on how to manage medications.
• Observes residents for changes and documents changes in their condition accordingly.
• Performs tasks associated with medication administration, including vital signs, medication dose calculations, stocking of the medication carts, and removal of discontinued medications.
• Practices safe handling and storage of medications.
• Responds to resident requests and other direct care duties as assigned.
• Identifies, manages, and treats specific skin conditions, such as pressure injuries; diabetic, venous, or arterial ulcers; and traumatic or complicated wounds.
• Provide wound care on assigned residents, in accordance with physician orders, following manufacturer recommendations and appropriate techniques.
• Completes a thorough and accurate wound assessment upon notification of wound. Completes follow-up assessments weekly and as needed. Documents all assessments in the medical record.
• Confers with the care plan team in the development of the comprehensive care plan.
• Documents resident care problems, abnormal findings, and changes in resident’s condition.
• Report to physician, supervisor, and resident/representative.
• Monitors implementation of preventive measures such as turning/positioning, pressure relief devices, nutrition/hydration measures, and toileting/incontinence care.
• Provides education to resident/representative as needed on topics concerning nutrition, preventive measures, and wound healing.
