Preferred Care Home Health is seeking a Full-Time Registered Nurse (RN) to provide skilled home health care to patients in assisted living, independent living, and other residential care communities in the Naples, FL area.

Qualifications

• Graduate of an accredited nursing program.
• Active, current Florida RN license.
• Current CPR/BLS certification.
• Minimum 5 years of RN experience.
• Home health and OASIS documentation experience preferred.
• Strong assessment, communication, and organizational skills.
• Comfortable working independently while collaborating with a team.

Responsibilities

• Perform comprehensive nursing assessments and admissions for home health patients.
• Develop and manage individualized plans of care alongside physicians and the care team.
• Deliver skilled nursing visits per physician orders and agency policy.
• Monitor patients closely and report changes in condition promptly.
• Educate patients, families, and caregivers on disease management, medications, and safety.
• Complete OASIS assessments and documentation accurately and on time.
• Coordinate with therapists, social workers, physicians, and facility staff.
• Maintain compliance with Medicare, CMS, and ACHC standards.
• Participate in case conferences and interdisciplinary team meetings.
