The RN will complete assessments, develop care plans, direct LPNs/CNAs, and deliver compassionate, patient-centered care in a home health setting.

Qualifications

Florida RN license in good standing.
Minimum 1 year nursing experience preferred.
Home health, hospice, or acute care experience preferred.
Current CPR certification.

Responsibilities

Perform comprehensive and ongoing patient assessments.
Complete Start of Care (SOC), recertification and reassessment visits.
Complete OASIS assessments accurately within timeframes.
Develop, implement and update individualized plans of care.
Provide skilled nursing services per physician orders and care plan.
Monitor patient condition and health changes.
Communicate changes to physicians and care team.
Coordinate care with physicians, therapists, LPNs, CNAs and others.
Provide direction/supervision to LPNs and CNAs as applicable.
Complete accurate clinical documentation in the EMR.
Ensure documentation supports medical necessity and regulatory requirements.
Participate in care coordination, team meetings and trainings.
