Job summary

Community Regional Medical Center in Fresno seeks a Registered Nurse (RN) Rehabilitation Clinical Liaison to join Signet Health in partnership. The role centers on pre-admission assessments and on-site care coordination outside the hospital walls.

Qualifications

• Current California RN license or allied health licensure (PT/OT/SLP) is required.
• Three years of experience as a clinical liaison in rehabilitation or closely related field.

Responsibilities

• Builds trust with patients, families, physicians and all referral sources.
• Proactively seeks referrals from hospitals, Assisted Living Centers, Home Health providers, physicians and case managers.
• Communicates daily with the ARU Medical Director and Director of Rehabilitation on referrals, admissions, and discharges, enters all required information in UDS.
• Develops and maintains positive relationships with hospital employees and Signet team members.
• Follows proper channels to present rehab education and unit criteria to case managers, physicians and other referring entities.
• Provides ongoing, follow-up and discharge communication to referral sources, patients, family, nursing staff, medical director, and therapists.
• Adheres to the hospital and Signet Health’s policies and procedures, ethical standards, corporate compliance standards and professional code of conduct.

Signet Health in partnership withCommunity Regional Medical Center in Fresno is looking for a dynamic RN as an addition to our Rehab Liaison team; this position is open immediately and has the potential to transition into a full-time role with benefits. This position will pay $80/hour.

The Clinical Liaison is responsible for pre-admission assessments in area hospitals and other local health care facilities. As the clinical face of a hospital based IRF, you will work primarilyoutside the hospital walls, you'll identify patients who need our level of care, conduct on-site clinical assessments, and build trusted relationships with referring physicians, case managers, and hospital teams.
