Job summary

Lifepoint Health is seeking a Registered Nurse (RN) for Acute Rehabilitation at our Chandler, AZ location. This part-time, day-shift role (2x12 hours, 6am–6pm) focuses on delivering high-quality patient care within a multidisciplinary team and coordinating discharge planning.

The ideal candidate will hold an Arizona RN license, with 6 months of Medical/Surgical experience preferred, CPR/BCLS required, ACLS or CRRN a plus.

Qualifications

• Must have current Arizona RN license (or Compact license with AZ endorsement).
• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
• Minimum six months Medical/Surgical experience in an acute care setting preferred.
• CPR/BCLS certification required; ACLS preferred; CRRN preferred.

Responsibilities

• Monitor and manage patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consult with nursing staff and multidisciplinary team regularly to evaluate patient’s status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitor patient and family satisfaction.
• Respond to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
• Provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

What We're Looking For

Applicants should have a current state RN license. Additional requirements include:

• Education: Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
