Position Overview

PRN/Per Diem position available covering areas of Central Brevard County. Trilogy Home Healthcare is a growing, privately owned home health agency serving over 42 counties across Florida with 10 branch offices. Responsibilities

• Provide the highest quality of care to patients in a professional and compassionate manner.
• Utilize all available resources to manage the medical needs of home health patients.
• Collaborate with interdisciplinary teams, including primary doctors and clinical managers, to optimize patient outcomes.
• Advocate for patients and communicate effectively with team members.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) of practice.
• Preferred: 1 year of nursing experience.
• Ability to exercise initiative and independent judgment.
• Acceptance of the agency's philosophy and goals.
