Responsibilities

The RN in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, following physician prescriptions and legal requirements. The RN coordinates care plans with healthcare professionals and ensures continuity of care through effective communication with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Deliver high-quality clinical services within scope of practice and infection control standards, according to the care plan, from admission through discharge.
• Complete clinical nursing assessments as per federal/state requirements and payer guidelines.
• Verify patient eligibility and medical necessity for services.
• Develop and update individualized care and service plans with other providers.
• Ensure care plans meet patient needs, initiating revisions with physician approval as necessary.

Education and Experience

Preferred: Minimum one year of RN experience. Must have current RN licensure in the state of practice, a valid driver’s license, vehicle insurance, access to reliable transportation, and current CPR certification.
