Job summary

Responsibilities

• Conduct thorough assessments and provide personalized, comprehensive home care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with Activities of Daily Living (ADLs) with dignity and respect.
• Coordinate schedule with patients per plan of care and preferences.
• Monitor changes in patient health and report to the care team.
• Administer medications and treatments as prescribed by physicians.
• Communicate with Case Managers and physicians for a unified care approach.
• Maintain accurate patient logs in the EMR system.
