Always Compassionate Health is seeking experienced, reliable RNs to provide one-on-one skilled nursing care to patients in their homes throughout New York. Qualifications

• Active NY RN license and current registration.
• Minimum of six months of home care nursing experience.
• Current CPR/BLS certification.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work independently and follow individualized care plans.
• Meet physical demands, including lifting up to 50 pounds.

Responsibilities

• Provide skilled nursing care per physician orders, individualized care plans, and agency policies.
• Administer prescribed medications and treatments and monitor patient responses.
• Perform G-tube/PEG care, tracheostomy care, and suctioning as required.
• Provide ventilator care when required and appropriately trained.
• Assess patient conditions, recognize changes in health status, and report concerns promptly.
• Maintain accurate, timely clinical documentation.
• Educate patients and caregivers on care needs, treatments, and safety practices.
• Follow infection control standards and established patient safety protocols.
