Job summary

Capital Caring Health is seeking a Per Diem RN to assess patient and family needs, use the nursing process, coordinate the Plan of Care with the IDG, and provide palliative and supportive care across the patient/family unit.

The role requires RN licensure, BLS, 1 year nursing experience (hospice/hospital preferred), EMR/EHR experience, and a valid driver’s license with reliable transportation to meet visit schedules. Compensation offered is $47.00 per hour.

Qualifications

  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.
  • Minimum one year nursing experience; hospice or hospital experience preferred.
  • Previous experience working with an EMR/EHR system.
  • Valid driver’s license and automobile insurance per company policy; reliable transportation to meet visit schedule.

Responsibilities

  • Provide and manage direct care to patients and families as part of IDG, incorporating psychosocial, spiritual, cultural, physical, and biological components.
  • Coordinate the individualized Plan of Care, ensuring it reflects evolving patient needs.
  • Educate patients, families, caregivers, and health professionals about disease process, palliative interventions, and safety practices.
  • Report changes in patient condition to appropriate members of IDG or other health professionals.
  • Participate with IDG to evaluate hospice referrals/admissions for level of care appropriateness.
  • Attend patient deaths, prepare bodies for transport, and oversee disposal of medications.
  • Provide bereavement interventions to family and caregiver with psychosocial staff.
  • Present concise and pertinent oral and written reports to IDG; encourage input from all disciplines.
  • Surveil all patient care provided by other nursing team members (LPNs, aides, contract nurses).
  • During emergencies, report to designated locations as required and potentially work beyond scheduled hours.
  • Perform other duties as assigned.

Job description

The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit.

Qualifications
  • Current license as RN in the state where the employee will be working
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross

  • Ability to use equipment with visual and auditory mechanisms
  • Effective communication in English (verbal and written)
  • Flexible schedule to meet the needs of Hospice House patients
  • Ability to manage emotional stress associated with caring for terminally ill patients and their families
  • Physical capability to access patient rooms and navigate patient homes, including stairs and narrow spaces, in any part of the service area
  • Ability to perform essential functions and physical requirements such as lifting patients, bending, pushing, pulling, and kneeling, with or without reasonable accommodation
  • For employees working at PACE: certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs
Additional Responsibilities
  • Cross training to hospice house or field positions when census calls for flexible staffing
  • Complete clinical admissions for all company services, including explaining services to potential patients/families, addressing concerns, completing informed consents, and initiating the Plan of Care
  • Initiate skilled nursing interventions to prevent complications and alleviate symptoms, and provide documentation per company policy
  • Act as the company representative at assigned facilities, facilitating referrals and communicating with admissions and after‑hours staff
  • Provide care for any GIP patients in a hospital, including daily visits, professional management, documentation, and efficient discharge
  • Develop strong relationships with case managers, physicians, and other facility staff; work with Professional Relations Representatives to educate on available services
  • Obtain consents for services, arrange safe discharge, and coordinate medical record information for referrals
  • Represent the company professionally at all times; maintain confidentiality and comply with all regulations and company policies

Physical Demands

Essential functions include seeing, hearing, talking, walking, and using hands to finger, handle, or feel. Frequently required to stand, sit, reach, lift, bend, and balance. Occasionally required to pull, push, stoop, kneel, and climb stairs.

$47.00
