Job summary

Job Overview

Registered Nurse (RN) provides quality nursing care via the nursing process, patient and family education, and documentation in a hospice setting.

Responsibilities
  • Provides and manages direct care to patients and families as part of an Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical, and biological components.
  • Coordinates the Plan of Care, ensuring individualized plans reflect evolving patient needs.
  • Educates patients, families, caregivers, and other health professionals about disease process, decline, palliative interventions, caregiving, dying process, and safety practices.
  • Reports changes in patient condition to appropriate members of the IDG or other health professionals.
  • Participates with the IDG to evaluate hospice referrals and admissions for level of care appropriateness.
  • Attends patient deaths, prepares body for transport, and oversees medication disposal.
  • Provides bereavement interventions to families and caregivers in collaboration with psychosocial staff.
  • Provides concise oral and written reports to IDG, encouraging input from all disciplines.
  • Communicates accurately and completely to physicians, staff, patients, families, and supervisors using positive approaches.
  • Supervises patient care performed by LPNs, Hospice Aides, and contract nurses.
  • During emergencies, may be required to report to designated locations and stay overnight as needed.
  • Performs additional duties as assigned.
  • Completes clinical admissions including explaining services, addressing objections, providing education, completing informed consents and assessments, recording physician orders, and coordinating plan of care.
  • Acts as company representative at assigned facilities, facilitating referrals, and maintaining timely communication with Admissions.
  • Provides all necessary clinical communication timely using SBAR and discusses potential needs with after-hours staff.
  • Obtains consents for services and arranges safe discharge efficiently while meeting productivity expectations.
  • Develops strong relationships with case managers, physicians, and other facility staff, and works with Professional Relations Representatives to provide education on available services.
  • Provides care for any GIP patients in hospitals, including daily visits, professional management, documentation, and efficient discharge.
Qualifications
  • Current RN license in the state of employment.
  • Active BLS from the American Heart Association or Red Cross.
  • Minimum 1 year nursing experience (hospice or hospital experience preferred).
  • Previous experience with an EMR/EHR system.
  • Valid driver’s license and automobile insurance per Company policy; reliable transportation to meet visit schedule.
  • Ability to use equipment with visual and auditory mechanisms.
  • Effective verbal and written communication in English.
  • Flexible schedule to meet the needs of Hospice House patients.
  • Ability to manage the emotional stress of caring for terminally ill patients and their families.
  • Physical ability to access patient rooms, other areas of the hospice house, hospital, or assisted living facilities, and to navigate stairs and narrow spaces, tolerate heat, and lack of air conditioning.
  • Ability to perform essential functions and physical requirements, including lifting patients and/or equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodation.
  • Employees working at PACE must complete certification of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
  • Some locations may require cross-training to hospice house or field positions of equal licensure/certification and scope when census calls for flexible staffing.
Physical Demands
  • Standing, walking, kneeling, reaching, lifting, bending, balancing, pulling, pushing, stooping/crouching, kneeling, and climbing stairs.
Compensation

Base pay rate: $10.00 per hour.

Compliance and Safety
  • Requires consent to drug and alcohol testing after a conditional offer of employment, and ongoing compliance with the Drug-Free Workplace Policy.
  • All employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse after a conditional offer, and ongoing eligibility must be maintained.
  • Terminals for equipment with visual and auditory mechanisms are required to be used safely.
