Responsibilities

• Promote patient and family-centered care, educate on illness management, and post-treatment care.
• Participate in safety, quality, and professional development activities.
• Develop and implement individualized care plans in collaboration with the healthcare team and patient/family.
• Assess physical, psychosocial, and emotional needs, respecting patient values and barriers.
• This is a safety-sensitive position requiring alertness and safe work practices.
