Job Description

The Registered Nurse (RN) is a member of the interdisciplinary team dedicated to meeting the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and their families facing terminal illness and bereavement. The RN is responsible for coordinating care, continuously assessing patient and family needs, and implementing the interdisciplinary plan of care.

Primary Responsibilities

  1. Manage cases and supervise nursing staff including Hospice Aides, LPNs, VNs, and volunteers as per the plan of care.
  2. Follow VITAS policies and standards, practicing within the scope of RN licensure and hospice eligibility requirements.
  3. Report any practice outside the scope of license to the supervisor.
  4. Perform comprehensive initial and ongoing assessments covering physical, psychosocial, spiritual, and bereavement aspects.
  5. Identify and utilize resources such as PT, OT, dietary, pharmacy, supplies, and medical equipment.
  6. Document all care activities accurately and promptly.
  7. Educate patients, families, and caregivers on care procedures, including pain management.
  8. Participate in patient care meetings and weekly team meetings.
  9. Provide effective communication through written reports, verbal updates, and body language.
  10. Engage in bereavement support activities.
  11. Contribute to on-call rotations and provide care during non-business hours as needed.
  12. Work collaboratively with the IDG to develop and follow the patient’s individualized care plan.
  13. Obtain physician orders when necessary and evaluate care outcomes.
  14. Delegate nursing tasks appropriately and monitor compliance with care standards.
  15. Ensure safety and proper delegation, especially for activities outside of standard scope, in accordance with state regulations.
  16. Participate in quality improvement initiatives, including data collection for quality measures and performance projects.
  17. Engage in ongoing professional development and training, including orientation and educational conferences.
  18. Serve as a patient and family advocate, addressing concerns and conflict resolution as needed.
  19. Represent VITAS positively within the community and in professional settings.

Minimum Requirements

  • Current nursing license in the state of practice.
  • At least two years of recent nursing experience in hospice, home health, or community health.
  • Reliable transportation with adequate insurance coverage.
  • Available telephone (cellular or landline).

Physical Demands

Must be able to assist with patient lifting and transfers, stand or walk for extended periods, and perform tasks requiring physical agility. Vision abilities include close, distance, and focus adjustment. Reasonable accommodations may be provided.
