Overview
The Registered Nurse Assessment Coordinator is responsible for the timely and accurate completion of the Federal and State assessment tool, The Minimum Data Set. Utilizes ADL's for resident assessments; defines appropriate criteria for determining patient status; works closely with the therapy department to determine the level, duration and type of patients' needs. Must be able to work with Occupational, Physical and Speech therapy and other departments in care planning and coordination of care. Demonstrates knowledge in Medicare reimbursement mechanisms.

Education and Experience
Graduate of an accredited nursing program.
BSN preferred.
Certification and Licensure
Licensed as an RN in state of practice, both WI and IL. Approved BLS certification required within 90 days
Skills and Abilities
Excellent interpersonal/communication and conflict resolution skills. Genuine concern for people. Strong verbal communication skills. Adept listener with the ability to interpret concerns. May include color blind test (per department guidelines)
