Job summary

St. Joseph of the Pines, part of Trinity Health Mid-Atlantic, is seeking a full-time Registered Nurse Manager/Clinical Care Coordinator to support resident-centered care in a Skilled Nursing Facility in Southern Pines, NC.

Reporting to the Director of Nursing, this role coordinates clinical services for an assigned population, oversees care planning, and collaborates with residents, families, and the interdisciplinary team to achieve positive outcomes.

Qualifications

• Graduate of an accredited School of Nursing.
• ADN or BSN required.
• Current Registered Nurse License in North Carolina.
• Minimum one year of recent long-term care or skilled nursing experience.
• Strong clinical assessment, communication, and leadership skills.

Responsibilities

• Complete resident assessments and develop individualized care plans.
• Coordinate admissions, discharges, and ongoing clinical care.
• Monitor resident outcomes and update care plans as needed.
• Administer medications and treatments in accordance with established policies.
• Provide clinical leadership, delegation, and support to nursing staff.
• Collaborate with residents, families, providers, and care teams to ensure quality care and resident satisfaction.
