Registered Nurse (RN) – Long Term Care

Schedule: 12-hour shifts as needed.

Responsibilities

• Perform initial and ongoing assessments of patient condition.
• Establish and implement a plan of care appropriate to identified problems and patient preferences.
• Deliver plan of care interventions, including medication administration, specimen collection and clinical treatments.
• Document patient care performed.
• Monitor, record and communicate patient condition to the care team, physician, patient and family.
• Assist patients with activities of daily living such as personal hygiene, elimination, nutrition, and ambulation.
• Collaborate across disciplines to coordinate patient care, including transfers, discharge, referrals and spiritual/psychosocial support.
• Identify patient and family learning needs and provide education appropriate to age, culture, condition and circumstances.
• Advocate for the physical and emotional well‑being of patients.

Qualifications

• Current Tennessee Registered Nurse license.
• Basic Life Support certification at the time of hire.
