The Registered Nurse will be responsible for demonstrating performance consistent with professional standards of practice, care, performance, the Nurse Practice Act and the mission and goals of the Community Health Network. The Registered Nurse manages patient care in the healthcare continuum to achieve optimum outcomes in a safe and cost-effective manner. The Registered Nurse demonstrates skills in collaboration, delegation, negotiation and management of resources. The RN on Transitions Unit will be responsible for completing admissions assessments (behavioral and physical), care plan management, patient education, medication administration and education, teaching therapeutic coping skills, and documentation. This unit will flex from adult to youth to support needs based on census and can include any diagnoses.
