Home Hospital Registered Nurse Overview
The Home Hospital Registered Nurse is responsible for providing the essential elements of the inpatient care delivery model into the patient's defined living space using leveraged technology and remote clinical care coordination. The RN delivers quality care through a team nursing model, documents care, and collaborates with a multidisciplinary healthcare team to deliver therapeutic interventions while maintaining high quality and safety standards.
Essential Job Functions
- Performs and documents the nursing process—assessment, diagnosis, planning, implementation, and evaluation—of patients receiving care remotely or in-person at their home.
- Reviews and follows up on patient education, implements patient care plans, and coordinates services.
- Triagees, identifies, and acts on changes in patient condition.
- Provides quality care during daily in-person visits for patients in the Home Hospital program, identifying patient needs through acute and in‑home assessments and implementing the medical treatment plan in collaboration with the 24/7 virtual nursing team and providers.
- Reduces avoidable readmissions and improves outcomes for patients enrolled in the program.
- Responds promptly to urgent in-person assessments as requested by the virtual nursing or provider team; performs diagnostics and treatments within nursing scope, communicating results and responses to the team.
- Coordinates appropriate hospital resources for acute inpatient individuals using Home Hospital services during their stay (e.g., therapy, consults, imaging, labs, transportation, medication administration, equipment).
- Provides phone triage and escalates appropriately using critical thinking skills.
- Utilizes and instructs patients and families on technology for biometric data capture, symptom reporting, and telehealth visits.
- Screen patients for continuum integration services prior to discharge, including remote monitoring and telehealth virtual visits.
- Provides bedside patient education and consultation on continuum integration services.
- Provides community resources to prevent unnecessary hospital admissions through collaboration with the interdisciplinary care team.
- Supports diversity, equity, and inclusion initiatives across the health system.
- Model personal safety practices and reliability organization principles.
- Assumes additional duties or special projects to support the VCUHS mission.
Patient Population
- Adult patients aged 18‑64 years.
- Geriatric patients 65 years and older.
Employment Qualifications
- Required Education: Diploma or Associate’s Degree from a professional nursing program recognized/approved by the state Board of Nursing.
- Registered nurses without a Bachelor of Science in Nursing (or higher) must enroll in an approved RN‑to‑BSN program within two years and complete it within five years, unless grandfathered.
- Preferred Education: Bachelor of Science in Nursing (or higher) from an accredited program.
- Licensure/Certification Required: Current RN licensure in Virginia or a compact state; American Heart Association Basic Life Support (BLS) HCP certification or equivalent.
- Licensure/Certification Preferred: Nursing specialty certification.
Minimum Qualifications
- Minimum of three (3) years of critical care, acute care, or medical/surgical RN experience, with at least one (1) year of RN experience.
- Follows documented physician/licensed independent provider orders.
- Practices within Virginia nursing regulations, ANA Code of Ethics, and VCUHS policies.
- Demonstrates cultural responsiveness through diversity, equity, and inclusion commitment, inclusive behaviors, and bias management.
Preferred Knowledge, Skills, and Abilities

Working Conditions
- Requires car travel to off‑site locations, sometimes in adverse weather.
- May encounter limited hazardous substances or body fluids; universal precautions and personal protective equipment are mandatory.
- Shift rotation and schedule determined by the Nurse Manager/Director; hours may be adjusted to meet unit requirements.
Physical Requirements
- Lifting/carrying 0‑50 lbs; pushing/pulling 0‑50 lbs.
- Physical activities: stooping, kneeling, squatting, balancing, bending.
- Occupational positions: sitting, walking, standing.
- Other tasks: shift work, maneuvering patient weight, hearing alarms/telephones/tape recorders, reaching above shoulder, repetitive arm/hand movements, finger dexterity, use of latex gloves, exposure to high‑pitched noises.
