Job summary

VHS Home Health Care is seeking an experienced RN to deliver skilled nursing care across multiple resident areas in Virginia. You will assess, document, and implement care plans while collaborating with a multidisciplinary team to ensure high-quality, patient-centered care.

Requirements include an active Virginia RN license, BLS/CPR certification, and 1–2 years in Home Health. The role offers full- and part-time options, with competitive hourly pay and comprehensive benefits.

Qualifications

• A current, unencumbered active license to practice as an RN in Virginia.
• Valid BLS certification (CPR) required.
• 1–2 years of Home Health experience required.
• Ability to manage care for multiple residents in a fast-paced setting.
• Strong clinical assessment, communication, and documentation skills.

Responsibilities

• Provide resident care according to standards and plan of care.
• Follow care plans developed by the Nurse Manager.
• Administer medications and treatments per POC.
• Monitor residents and report changes to the clinical team.
• Maintain accurate documentation per orders and policies.
• Collaborate with interdisciplinary teams for coordinated care.
• Ensure infection control and safety regulatory compliance.

Guided by our core values of Passion, Respect, and Excellence, the Registered Nurse (RN) is responsible for delivering skilled clinical care, monitoring resident conditions, and ensuring all care is provided in accordance with established standards and individualized care plans.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician's plan of treatment, as well as the written plan of care.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments in accordance with the plan of care (POC).
• Monitor and assess residents' conditions, promptly reporting changes to appropriate clinical team members.
• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.
• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.
• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of Virginia.
• Valid BLS Certification (CPR) required.
• 1-2 years of previous Home Health experience required.
• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.
