We Are Hiring
  • Department: Hospice Therapeutic Services
  • Schedule:12-hour DAY Shift - Part time
  • Facility: Home Health and Hospice
  • Location:Hanover Park, IL
  • Salary: $37.15 - $52.44 per hour
What You Will Do

Coordinate patient intake activities and resources.

  • Evaluate severity of illness for all elective, direct, and urgent admissions to determine appropriate level of patient care.
  • Review physician orders for clinical appropriateness/completeness and proper admission status.
  • Coordinate education and development of internal staff and external departments to ensure understanding of intake review and approval process.
  • Conduct complex clinical review for inpatient, outpatient, and ancillary services requests for medical necessity.
What You Will Need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.

Education:

  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
Additional Preferences
  • 2+ years of nursing experience strongly preferred

  • 1+ year of Hospice experience preferred
