VitalCaring Group is seeking a Hospice Registered Nurse (RN) to lead one-on-one, field-based care in homes and community settings. You’ll manage the plan of care, monitor symptoms, and partner with patients and families to honor goals with comfort and dignity.

You’ll collaborate with an interdisciplinary team, provide education and emotional support, and document care with accuracy while maintaining compliance. This role emphasizes autonomy with strong team backing across care settings.

Qualificações

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Lead Patient-Centered Hospice Care: conduct comprehensive assessments, develop and manage individual care plans, deliver skilled nursing with focus on pain and symptom management.
• Serve as the Care Coordinator: act as case manager, coordinate care with the interdisciplinary team, monitor patient condition and adjust plans as needed.
• Support Patients & Families: provide education, emotional support, and guidance, help navigate decisions with empathy and clarity, build trust.
• Drive Quality & Compliance: maintain accurate documentation, participate in IDT meetings, support quality standards.

Role Overview

What You’ll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments

• Deliver skilled nursing care with a focus on pain and symptom management

What You Bring
