Phoenix Home Care & Hospice in Kansas City, Missouri is seeking an experienced Hospice Admissions RN to guide patients and families from curative care to comfort-focused care with compassion and clinical excellence. You’ll collaborate with intake coordinators, medical directors, facility staff, and physicians to ensure admissions are smooth and centered on what matters most.

Responsibilities

• Complete hospice admissions and skilled nursing visits.
• Build meaningful, day-to-day connections with patients and families.
• Provide education on conditions such as CHF, COPD, and Diabetes.
• Perform wound care and medication management.
• Change dressings and medical devices as ordered.
• Accurately document patient conditions and communicate changes to physicians.
• Partner closely with interdisciplinary hospice teams to deliver seamless care.

• Full Time: 40hrs | 10-hour Shifts | Monday - Friday
