Job summary

Transitions Care seeks a compassionate Registered Nurse to provide hospice care in home and facility settings. You will deliver skilled nursing with focus on symptom management, comfort, and quality of life for patients with serious or terminal illnesses.

You will complete assessments, create and update care plans, educate patients and families, and collaborate closely with an interdisciplinary team to support dignity and end-of-life care.

Qualifications

• Must hold a valid RN license.
• Experience in hospice care is preferred but not required.
