Job summary

Transitions Care is seeking a compassionate Registered Nurse to provide hospice visit care in patients’ homes and facilities. You’ll deliver skilled nursing assessments, manage symptoms, administer medications, and implement individualized care plans focused on comfort and quality of life.

You will educate and support patients and families, coordinate closely with an interdisciplinary hospice team, and ensure timely documentation.

• Skilled nursing assessments and symptom management for hospice patients.

• Strong clinical judgment and excellent communication in end‑of‑life care.

Responsibilities
