Job summary

Transitions Care is seeking a compassionate Registered Nurse to provide hospice visit care in patients' homes and facilities. You will perform skilled nursing assessments, manage symptoms, administer medications, and implement individualized care plans focused on comfort and quality of life.

You will educate and support patients and families, coordinate with an interdisciplinary hospice team, and ensure timely documentation to uphold dignity and holistic end-of-life care.

Qualifications

• Active RN license in Illinois or eligible for licensure.
• Hospice or home health experience preferred.
• Strong communication and clinical judgment in end-of-life care.
