The Weekend Home Health RN role at Interim HealthCare - Topeka KS delivers patient-centered care in Topeka, KS, with 1:1 patient ratios and weekend OASIS visits.

You will perform SOC/ROC/recertifications, develop care plans, coordinate with physicians and therapists, ensure Medicare compliance, and maintain thorough documentation, all while joining a family-owned team that values growth and support.

Qualifications

• Graduate of an accredited nursing program with an active RN license in Kansas.
• CPR certification and knowledge of home health regulations.
• Strong communication and compassionate patient care.

Responsibilities

• Complete accurate OASIS SOC/ROC/Recertification/Discharge assessments.
• Develop, implement, and update individualized patient care plans.
• Perform skilled nursing visits including assessments, meds, wound care, patient education.
• Coordinate care with physicians, therapists, and interdisciplinary team.
• Ensure compliance with Medicare, state, and agency regulations.
• Maintain timely clinical documentation in the EMR.
• Provide education and support to patients and caregivers.

As a Home Health RN Weekend Nurse, here’s a big-picture view of what you’ll do:

• Complete accurate and timely OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments

• Develop, implement, and update individualized patient care plans

• Perform skilled nursing visits including assessments, medication management, wound care, and patient education

• Coordinate care with physicians, therapists, and interdisciplinary team members

• Ensure compliance with Medicare, state, and agency regulations

• Maintain thorough and timely clinical documentation in the EMR

• Provide education and support to patients and caregivers

A few must-haves for this position:

• Graduate of an accredited nursing program and active RN license in Kansas
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
