Interim HealthCare is seeking a Weekend RN in Topeka, KS, to provide home health care with OASIS visits, care planning, and patient education. The role emphasizes advocacy, quality outcomes, and collaboration with the care team in a patient-centered setting.

Formación

• Graduate of an accredited nursing program and active RN license in Kansas.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing with strong communication and compassion.

Responsabilidades

• Complete accurate and timely OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments.
• Develop, implement, and update individualized patient care plans.
• Perform skilled nursing visits including assessments, medication management, wound care, and patient education.
• Coordinate care with physicians, therapists, and interdisciplinary team members.
• Ensure compliance with Medicare, state, and agency regulations.
• Maintain thorough and timely clinical documentation in the EMR.
• Provide education and support to patients and caregivers.

As a Home Health RN Weekend Nurse, here’s a big-picture view of what you’ll do:
